NonEmployee Terms Enrollment: By completing the following form, you are requesting enrollment in a class. You will receive confirmation from Katie Hedrick, Human Resources & Training Coordinator, within 24 hours. You may enroll up to 24 hours prior to the beginning of class. Enrollment will be handled on a first come, first serve basis. Cancellation: Cancellations must be made within seventy-two (72) hours prior to the start of class. Cancellations must be confirmed with Katie Hedrick. Payment: Upon receipt of your training request, Katie Hedrick will contact you if payment is required. If course requires payment, checks should be made payable to Bello Machre, Inc and mailed to the attention of Katie Hedrick at 7765 Freetown Road, Glen Burnie, MD 21060. Please note the participant’s name and the date of the course in the “memo” section of the check for reference. Payment may also be made via credit card by calling Katie Hedrick at 443-702-3038 and providing the applicable information. Payment should be made in full prior to the start of the course. Instructors are not able to accept payment. There will be a $25.00 service charge for any returned check. A full refund will be issued when a participant withdraws prior to the required withdrawal period if the participant already made payment. If a course is cancelled by Bello Machre, participants will have the opportunity to reschedule or to obtain a full refund. All refunds will be made via check, made payable to the original payer. Other: In the case of inclement weather, please call Katie Hedrick at 443-702-3038 for cancellation information. Dress code is appropriate casual clothing.
